REGISTRATION INFORMATION
• Regional Entry Form MUST be filled out and paid for at the State Meet.
• Forms will be made available at the State Meet.
• Please be prepared to submit one check per club, per level BEFORE leaving Regionals.
• Make sure that any Regional Injury Petition Gymnasts are listed and paid for on your Regional form.
• Make checks payable to: Twistars Booster Club
• $80 per gymnast
• $25 Late Fee per gymnast
